HOW HOSPICE WORKS
Hospice Care goal is palliative rather than curative.
The patient, family member or friend can request or directly call for Hospice care. Physicians, clergy or other health professionals can call for Hospice care upon request or conferring with the patient and/or family that Hospice will benefit the patient. If the patient desires, Essenthial Home Health can also talk with the patient's physician to determine the patient's eligibility for hospice care

When the patient and/or family decides Hospice care will be beneficial, a nurse initiates the services by visiting the patient and does an initial assessment. Once we start our service, our Interdisciplinary Care Team members will then visit the patient to provide care and services based on the Plan of Care that is individualized for the patient.

Typically, a family member or a close friend serves as the primary caregiver and when appropirate, helps make decisions for the terminally ill cared one.
