Overland Park, KS. - Essential Home Health ,Inc. proudly announces its award of accreditation status by the Accreditation Commission for Health Care, Inc. (ACHC) for the services of Home Health and Hospice.

ACHC, a private, not-for-profit corporation which is certified to ISO 9001:2000 standards, was developed by home care and community-based providers to help companies improve business operations and quality of patient care. Referring to the value of accreditation, ACHC President Tom Cesar indicated, "The survey process leads an organization to examine its policies and practices continually to clarify its strengths and improve its weaknesses."

Accreditation is a voluntary activity where healthcare organizations submit to peer review of their internal policies, processes and patient care delivery against national standards. By attaining accreditation, Essential Home Health, Inc. has demonstrated its commitment to maintain a higher level of competency and strive for excellence it its products, services and customer satisfaction.

Essential Home Health, Inc. provides a one-stop -shop for patients who need Home Health, Hospice or Private Duty services. The Owners are locals to the Kansas City area and are both Registered Nurses who have been in the healthcare business for over 15 years. The Interdisciplinary Team include the Medical Director, RN's, Home Health Aides, PT's, OT's, Speech and Respiratory Therapists, Social Worker, Dietitians, Chaplain and Volunteers. Our Director of Patient Care Services oversees the Care Planning of all Teams involved with Patient Care. The Care Team Members include ;
A) Skilled Nursing ( RN's, LPN's)
B) Home Health Aides ( bathing and other personal care, patient must have another skilled care needed like Nursing or PT to receive this service)
C) Rehab Therapists ( PT, OT, SPEECH)
D) Dietitian ( assess patient's nutritional status to support the recovery process)
E) Medical Social Worker ( assesses social, economic, safety concerns, counseling, Assistance with Community Resources)

COLLABORATION OF CARE

The Care Team Members meet on a regular basis to discuss Plan of Care of individual patients in relation to the patient's progress in reaching the discharge goals. Each Team Member reports their individual assessments and observations and collaborates closely with the patient's Physician, any Specialty Clinics and the patient's Family Members.
The whole Care Team work cohesively under the Plan of Care specifically made for each patient.
